Registration & Enrollment

REGISTRATION for currently enrolled students:
Attending In-Person Registration for currently enrolled students* is NOT required (as stated in the EACS postcard).  Leo Elementary only requires on-line registration through Skyward Family Access.
*Note: If your child attended Cedarville Elementary last year, they are considered a currently enrolled student.
Online Registration for currently enrolled students will begin on July 23, 2021.

For families who need to enroll NEW students, please attend in-person Registration on:
Wednesday, July 28, 2021 12:00 pm to 7:00 pm AND Thursday, July 29, 2021 8:00 am to 12:00 pm

ENROLLMENT for new students:
To enroll in-district new students to Leo Elementary please contact Lisa Kern, Registrar/Secretary, by phone at 260-446-0170 to inform the school of your intent to enroll your child and the grade level they will be entering. Then complete Steps 1-3 below.

School Office Hours: Monday- Friday, 7:30 am – 4:00 pm

Summer Office Hours & Dates:  The school office will be closed for summer break beginning June 21, 2021 and will re-open on July 26, 2021. 

School Tours:  The school will be open for you to tour during Registration Days on July 28 (12-7 pm) & July 29 (8 am - noon).  All other dates, please contact the school office for an appointment.

Step 1:

Step 2: Provide the following documentation:

  • Original Birth Certificate
  • Proof of Guardianship - Legal Documents or Court Order if other than parent
  • Immunization Records - Doctor's office can fax to 260-446-0173, Attn: Registrar
  • Two (2) forms of Verification of Legal Residency, such as:

1.  Current utility bill (gas, water, electric, phone, etc.)
2.  Computer generated bill issued by Doctor or Hospital
3.  Letter or check stub from any governmental agency
4.  Court Documents
5.  Lease printed on letterhead from an established apartment/Mobile home leasing office with acontact name and phone number.  Hand written leases or those printed from the internet are not acceptable.
6.  Mortgage document or statement

NOT acceptable as proof of residency:  cell phone bill, cable bills, magazine subscription mailing, items mailed to a PO Box.

NOTE: Residency means a permanent & principal place of habitation, which a person uses for a home, for a fixed or indefinite period & remains when not called elsewhere for work, studies, recreation, or other temporary or special purposes.  A temporary or occasional dwelling used primarily for school attendance purposes i not a Legal Residence.  A student may be expelled (excluded) from school if the student's legal Residency is not within the EACS boundaries.

STEP 3: Please be sure to sign and date the enrollment form and submit via email to [email protected] with all the required documents, or make an appointment to submit the forms in person by calling 260-446-0170.
