Parents/guardian of new students to Leo Elementary School will need to call or stop by the school office as soon as possible to fill out enrollment papers.  Office hours are from 7:30am - 4:00pm. through  June 18, 2019. Office will re-open July 29th  .  It is helpful to call the office first so we can schedule an appointment and provide a tour of the school.

Parent will need to provide the following in order to enroll their child:
  • Original Birth Certificate
  • Proof of Guardianship - Legal Documents or Court Order if other than parent
  • Immunization Records - these may be faxed from doctor's office (fax #260-446-0173)
  • Two forms of Verification of Legal Residency

             1. Current utility bill (gas, water, electric, phone, etc.)
             2.  Letter or check stub from any governmental agency
             3. Court Documents
             4.  Lease printed on letterhead from an established apartment/Mobile home leasing office with a contact name and phone number.  Hand written leases or those printed from the internet are not acceptable.
  NOT acceptable as proof of residency:  cell phone bill, cable bills, magazine subscription mailing, items mailed to a PO Box etc

            Note:  Residency means a permanent & principal place of habitation, which a person uses for a home, for a fixed or indefinite period & remains when not called elsewhere for work, studies, recreation, or other temporary or special purposes.  A temporary or occasional dwelling used primarily for school attendance purposes i not a Legal Residence.  A student may be expelled (excluded) from school if the student's legal Residency is not within the EACS boundaries.
